Through the partner referral program, those needing help can take advantage of the many great services both CAC and Greater Health Now have to offer. GHN helps people access the care and resources they need to get, stay, and be healthy.
Our Community-Based Worker and Greater Health Now can:

- Bring organizations together to improve overall community health
- Help connect people to services like healthcare, housing, food, and transportation
- Support programs that address social needs (not just medical care).
- Offer things like health insurance enrollment assistance and care coordination.
- Invest in and fund local community-based programs.
